Referral Form

Please note, that a veterinary referral form must be signed and sent to HCHR prior to your first appointment with us. This is a legal requirement under the Veterinary Surgeons Act. It is preferred that this is accompanied with relevant patient history. We can email the referral form to your veterinary surgery for you, or if preferred you can simply download it using the links below and arrange this yourself.
